I am a British national, resident in the United States. I work freelance, and am paid from a variety of different countries. My current main bank - which is English - charges me fees for a whole variety of services (transferring money, withdrawing money when I am not in the United Kingdom).
I would like to open an account which allows me to: withdraw money without exhorbitant charges anywhere in the world (I am frequently in Africa), and allows me to transfer money between accounts and currencies without excessive costs. It could be an account in the Netherlands (where I am presently resident), the United Kingdom, or the USA (where I will be resident). Any suggestions?

There are a large number of banks in the U.S. that provide very inexpensive service for routine transactions. Some offer free ATM transactions, and some even reimburse customers for fees imposed by other ATM owners. It is standard to impose wire transfer fees for moving funds between accounts in different banks, particular between countries.
The service you appear to need is well beyond what most banks would consider routine, so it would be unreasonable to expect real bargains on service costs. To get the features you want, you may need to deal with a larger bank with an international banking department. Such banks tend to command higher fees.
