I'm in AZ and my 84 year old uncle is in NY. He wants me to be on a joint account for his two bank accounts. I was told he needs to do a Power of Attorney form and get it notarized. Does his bank in NY arrange the paperwork? What would I need to do on my end in AZ?

First, your uncle and you need to resolve what is to be done. Does he want you to be added to his accounts as a joint owner, where you would have equal access to the funds and would receive the funds as surviving owner in the event of his death, or does he want you to sign on the account under the authority of a power of attorney, where you would act on his behalf to manage the account, but the account would be part of your uncle's estate on his death, and you would have no further access.
If a power of attorney is to be used, your uncle should discuss his needs with an attorney.
Once you have resolved the course your uncle wishes to take, he should contact his bank in New York for guidance on how to proceed, perhaps with your help.
