If a bank returns a charge nonpaid, how can they charge you an overdraftfee when your account never really overdrafted?

If the fee is for the processing, and not for the payment of an item, yes. If I have $100 in my account and a check or other transaction is presented for $101, that request can be denied. I would be charged an NSF fee of say $25 and would subsequently have $75 left in my account, as well as someone still wanting $101 from me.
