I am the treasurer for my son's newly-formed baseball team. Can I open a checking account for the team in my name or does it have to be in the name of the baseball team? If I set it up as a business account, what additional forms do I need to fill out?

Ideally, the team will apply for its own taxpayer identification number from the IRS. That will enable you to open the account in the name of the organization, rather your own. There will come a time when you'll be surrendering the books to someone else. When that time comes, you will be glad the account isn't in your name.
It's likely, however, that the team won't get incorporated as a separate legal entity. That will mean that a bank will need to record your name, social security number, address and date of birth, even if the account is in the team's name.
Banks vary widely in the amount of documentation they require when setting up accounts for groups like this. Your best bet would be to contact the bank where you do business and ask what they will need from you to get this new account set up.
