I have a joint account with my mother and she wishes to remove her name from the account. How do I go about doing so?

At most banks, the preferred method would be for you to close the account out and open another account in your name alone. That provides a clean separation of the accounts and prevents any confusion. As one of the joint account owners, you should be able to accomplish the transaction without any need for your mother's involvement.
