You can attempt to contact the bank that issued it. If the bank no longer exists, it's possible its deposits were assumed in a merger or acquisition. You can research what happened to the bank and obtain contact information using the FDIC's "Bank Find" service.

Once a CD is paid out, a bank does not have to maintain records of the old account. Five years is the minimum requirement, and if an account remains unclaimed for a number of years (two or more, depending on the state), state law on abandoned property may compel a bank to surrender the funds to the state's unclaimed property offices.
