About the Monthly Bank Statement

Do the names of both owners or individuals have to appear on the monthly statement that is mailed out, or can only one name appear on the statement?

What matters is the contract between the depositors and the bank, and the records of account ownership at the bank. While it is usually the case that joint owners' names both appear on statements, it's not a requirement, and when more than two owners share an account, it can become impossible for a bank to list all the names.
