Is it ethical of my banker to say to me "Take your money out of 'Bank A', they are failing." It didn't make me want to move any more money to his bank, I felt it was at least unprofessional.

This "banker" has no more information that you do from various public sources on Bank A. It may be unprofessional, especially if the motive was to get you to move your accounts to his bank.

My father had a person as the power of attorney, the bank did not have a signed power of attorney form, according to a mn lawyer, if it wasnt signed it needed to state that they had seen the signed copy with date. We are located in MN

Also would it be considered forgery if someones name is on the account (only that name on the checks) and that person writes checks and signs the other persons name?

Person A cannot sign person B's name without legal approval.
