Does a bank have to carry an affidavit of fraud form?
Most banks can provide their customers with standard forms that can be used to complete and file routine claims of forged or counterfeit checks or forged or unauthorized endorsements. Additional forms are also likely to be available for claims of unauthorized electronic fund transfers, particularly those completed through the automated clearing house (ACH) system. Completion of other than these routine claim forms might require the use of an attorney. Other than the ACH-related forms, often referred to as a Written Statement of Unauthorized Debit (it's not a true affidavit), there's no actual requirement for a bank to supply these forms.
